Spring 2010 Registration

Spring Season will begin on April 10th thru June 10th.

Registration is an on-line process, please click on the link and it will take you to the required form. If you do not have a computer, you can come to the office during office hours to fill out forms.  Office hours will begin January 9th.  The office will be open Tuesday's 6pm to 8pm and Saturday's 10am to Noon.

Uniform orders must also be done on-line. Please click on the link and it will take you to the required form.

Medical release forms are required for tournaments. This form must be turned into your Coach/Manager. It does requiry a notary. If you need a notary, please contact Michelle Jones at southlyonsoccer@gmail.com
