First Registration Form US citizens born outside of the US who have never been registered to play soccer in another country

Entry prior to 12 form Players who entered the US prior to age 12. Must supply a copy of an official document to prove that fact

International Clearance for Minors Any player currently between the ages of 12 to 17 who is NOT eligible for one of the proceeding methods must prove to US Soccer that he/she meets one of these two exceptions: 1) The player has moved with his/her parents to the US for reasons other than playing soccer, OR 2) The player and prospective club are both located within 30 miles of an international border.
